CMS Scores Required for Nursing Home Five Star Ratings
The table below shows the maximum ratings score required to qualify for each of five stars in the Five Star Rating System for Nursing Homes developed by the Center for Medicare and Medicaid Services. For purposes of this system lower scores are better. For each state, CMS divides the nursing homes by score so that the top ten percent of the nursing homes in the state are given five stars and the bottom twenty percent are given one star. The each of the middle levels represents approximately 23% of the nursing homes in the state.
